ABSTRACT
Patología frecuente en nuestro medio, que se debe principalmente a infecciones pulmonares por Staphylococcus, Streptococcus, o Haemophilus; suele presentarse en menores de 6 años y se caracteriza por: fiebre, tos, dolor torácico, insuficiencia respiratoria, hipoventilación parcial o total de un hemitórax y percusión mate; suele cursar con leucocitosis a expensas de neutrofilia radiografía de tórax con opacidad marginal o total de un hemitórax. Requiere de manejo antibiótico, punción evacuadora o colocación de una sonda pleural y en ocasiones tratamiento quirúrgico

To document the normal values of pulmonary function tests in children and changes occurring in their values with various respiratory disorders, a study was carried over a period of one year in 95 healthy controls (39 females and 56 males) of 8-13 years of age and 51 cases with respiratory disorders (bronchial asthma-31, pneumonia-10, empyema-10) of matched age, sex and height distribution. The lung functions studied were FVC, FEV1, FEV1/FVC, PEFR and FEF25-75%. In children with bronchial asthma, the FEV1/FVC%, PEFR and FEF25-75% were reduced in accordance with the severity of the disease. A typical restrictive pattern of equivalent decrease in FVC and FEV1 along with insignificant lowering of flow rates, i.e., PEFR and FEF25-75% was observed in pneumonia whereas in patients of empyema a combined pattern of significantly decreased FVC and FEV1 along with mildly reduced FEV1/FVC%, PEFR and FEF 25 75% was observed.

El empiema pleural es un cuadro siempre secundario que evoluciona desde una fase exudativa, pasando por una fibrinopurulenta hasta llegar a una de organización. Su diagnóstico y tratamiento precoz se relaciona con un alto porcentaje de mejoría y bajo índice de morbimortalidad y complicaciones tardías, lo cual se invierte a medida que se produce demora en el diagnóstico y tratamiento. Por este motivo se describen los principios básicos de su tratamiento a través de las diferentes fases, que en términos generales consisten en un adecuado drenaje de la cavidad pleural, buena expansión pulmonar y recuperación de la función respiratoria. También se hace un breve alcance sobre algunas condiciones especiales relacionadas con el empiema pleural
